Indonesia recorded its highest malaria caseload in 2025, with infections surging in Papua and reappearing in previously malaria free districts, as climate change, population mobility and tighter budgets strain elimination efforts.

Record Caseload And Shifting Patterns
Health Ministry data show malaria cases rising from 418,000 in 2023 to 543,000 in 2024 and 706,000 in 2025, the highest level in recent years. Communicable diseases director Prima Yosephine said fluctuations are largely driven by population mobility and weather conditions, with 412 of 514 regencies officially malaria free but small pockets and renewed outbreaks pushing national numbers upward.

Papua’s Geography And Funding Constraints
More than 95 per cent of cases now occur in six Papuan provinces, with the highest concentration in Papua province’s vast forested and swampy areas where Anopheles mosquitoes thrive and communities are hard to reach. Prima said elimination there is hampered by difficult terrain and reduced funding after President Prabowo Subianto’s austerity measures, which have cut malaria budgets and forced heavier reliance on philanthropic support for costly outreach and logistics.

Reintroduction Through Migration And Climate Pressures
Outside Papua, infections are often reintroduced by people moving from endemic zones, as seen in Rokan Hilir, Riau, which declared itself malaria free in 2018 but saw cases rebound to over 2,000 in 2024, prompting repeated local emergencies. Prima also linked the wider surge to climate change, with rising temperatures and erratic, heavy rainfall creating more breeding sites and extending transmission seasons, raising risks even in areas that previously had low incidence.

Current Responses And Targeted Interventions
To stay on track for the 2030 elimination target, the Health Ministry is strengthening surveillance, expanding free antimalarial treatment, intensifying mosquito control and promoting behavioural change such as repellents, window screens and bed nets. Targeted actions in high risk groups include net distribution and preventive treatment for miners, forest workers, police and military personnel, alongside efforts to tighten monitoring in mobile populations and remote settlements.

System Gaps And Long Term Risks
Epidemiologist Dicky Budiman warned that the 2025 spike exposes deeper system gaps, including weak integration between health, environmental, energy and labour policies, and the impact of unregulated logging and mining that alter ecosystems without proper health impact assessment. He cautioned that failure to meet the 2030 goal would threaten national health security through recurrent local outbreaks and possible regional spread, while raising economic costs through lost productivity and mounting healthcare spending.

Indonesia’s surge in malaria cases highlights how climate stress, mobility and budget cuts can undermine hard won gains against a disease that still hits eastern regions hardest. Indonesians need sustained, better funded and more integrated strategies that link health with land use and climate adaptation, while Singaporeans share an interest in strong malaria control next door to reduce cross border risks, protect regional workers and support stable, healthy growth across South East Asia.
